The MYHA 2008-2009 hockey season is fast approaching and this year there is a new process to register your hockey player and coaches. MYHA is requiring all players and coaches to register on-line with USA hockey (http://www.usahockeyregistration.com). In the past these fees ($30 USA hockey fee and the $10 MN hockey fee) were included as part of your registration fees, and MYHA filed these on your behalf.
To register go to the link below and follow the prompts: register online, ice player and coaches, confirm participation and register for the 2008 – 2009 season. Please give your child’s legal name as listed on his/her birth certificate. PLEASE DO NOT USE NICKNAMES.
Once you have entered all the required information, you will receive a confirmation page that you MUST turn in with your registration paperwork. This confirmation will replace the Waiver of Liability form. This confirmation number must be printed and included with your MYHA registration forms.
USA Hockey Fee’s:
Players Born in 2000 and before
USA Hockey Fee -- $30 per player. MN Hockey Fee -- $10 per player.
Players born in 2002 and After
USA Hockey Fee -- No Fee. MN Hockey Fee -- No Fee
Coaches
USA Hockey Fee -- $30 per Coach. MN Hockey Fee -- No Fee

USA Hockey Registration Required for All Coaches and Players Prior to MYHA Registration
MYHA 2008-09 Walk-in Registration August 23, 2008
The walk-in registration and equipment swap will be held on Saturday, August 23rd from 8:30 – 11:30 in the Middle School Commons area.
If you are unable to attend the registration session, then registration forms may be mailed to:
MYHA Registrar, P.O. Box 604, Willernie, MN 55090.
Forms must be postmarked by August 23rd or a late fee will incur.
Please download the following two forms to complete your registration:
1. 2008-2009 registration form
2. Consent to treat
3. Birth certificate – new members only
4. Printed copy of USA Hockey player registration with number
